Program Overview
The House Calls Program (known as @HOME) delivers high-quality primary care to Medicare patients in their home. The @HOME program takes a team-based approach to providing outstanding patient care. Our Care Teams - consisting of a Provider, Medical Assistant/Scribe (CIS/MA), Behavioral Health Specialist (BHS) and Mobile Operations Coordinator (MOC) - build individual relationships across a small panel of patients, seen monthly. The Care Team is responsible for delivering excellent, high-touch primary care, and coordinating the care of patients.
Role Description
The House Calls Lead Behavioral Health Specialist (BHS) reports to and collaborates with the market Medical Directors in driving BHS operational initiatives, supporting continuous improvement projects, liaising with behavioral health and medical social worker leads within the broader organization and continues to manage their own panel of patients in a traditional House Calls BHS capacity.
Core Responsibilities
- Function as the behavioral health and social service expert to regional care teams
- Actively participate in idea generation and problem solving discussed during daily huddles and weekly interdisciplinary team meetings
- Cascade programmatic information to regional BHSs, as well as escalates care team concerns and ideas to program leadership
- Lead weekly BHS interdisciplinary case discussions
- Lead the execution of continuous improvement initiatives
- Ensure regional operations are running smoothly and safely on a daily basis, including ensuring there is adequate staffing each day, arranging coverage for any call-offs or scheduled PTO, ensuring scheduling and billing processes are executed efficiently, ensuring that all Standard Operating Procedures are followed, ensuring BHS documentation is meeting service standards
- Act as an ambassador with social service and community organizations in the region
- Support evolution and delivery of BHS onboarding and mentoring new hires through the first 120 days of employment.
- Manage patient panel in a traditional House Calls BHS capacity.

What we're looking for
- Genuine passion for primary care and reaching vulnerable patients
- Comfort navigating visits in patient homes
- 3+ years of experience providing counseling and social services in patient homes
- Demonstrated ability to collaborate effectively in a team setting
- Comfort with supporting BHSs who are managing patients facing significant health, psychosocial and/or mental health issues
- Excellent communication, follow-up, teamwork, and problem-solving skills
- Desire to be a part of a fast-paced, innovative, quality-driven organization
Required Qualifications
- Master of Social Work degree required from a school of social work accredited by the Council on Social Work Education (CSWE)
- Clinical Social Worker License (or local equivalent)
- Experience performing comprehensive biopsychosocial health assessments, mental health and substance abuse disorder assessments
- Experience with assessment and treatment planning for those with common mental health and/or substance use disorders and complex psychosocial needs
- Working knowledge of differential diagnosis and psychosocial treatments of common mental health and/or substance use disorders
- Proficiency in providing brief, structured intervention techniques (e.g., Motivational Interviewing, Behavioral Activation, Cognitive Behavioral)
- Computer skills: Ability to quickly navigate and use multiple computer programs to include, but not limited to: Gmail, MS Word or Google Docs, Excel or Google Sheets, etc.

Pay Range
The typical pay range for this role is:
$54,095.00 - $142,576.00
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
